Job Description: The Home Care Practice Manager maintains twenty four hour accountability for the delivery of patient services and clinician support in compliance with all regulatory/licensing standards and the hospital mission. The Practice Manager insures adherence to standards of care and practice to provide positive patient outcomes, achieve budget goals, and insure quality, cost-effective services for all customers. The Practice Manager manages, plans, and supervises Home Care Clinical Operations, ensuring clinical staff competence and evaluating and/or improving the effectiveness of the processes, systems and programs which support both patient care and staff performance. He/She supervises RNs, CNAs and the Schedulers, as well as serves as a clinical resource/liaison for other members of the Home Care Team. In collaboration with other departments, disciplines, and/or contracted services, he/she actively engages in Home Care program planning and evaluation. The Practice Manager oversees the delivery of Home Care services by all members of the interdisciplinary team and provides Home Care Nursing care or patient consultation when necessary to members of the interdisciplinary health care team (HCT). Hours: 1st Shift Education: Diploma in Nursing and a Bachelor’s in a health related field required Experience: Master’s preparation in Nursing or Management preferred. Knowledge of JCAHO & Medicare standards w/ advanced education/certification in home care nursing preferred. Home Care nursing experience preferred, with minimum of three years relevant clinical experience required. Previous management experience within a home health care environment preferred License: Rhode Island and Mass RN License required. Current BCLS certification required Knowledge, Skills & Abilities: Demonstrated clinical competence with physical assessment and critical thinking skills.
